Instructions to fill up forms

1. The applicant must submit a perfect application with appropriate supporting proofs to the Commission

2. It should be noticed that, the application is filled up keeping harmony with this rules.

3. The application for registration will be considered if it is perfect in all respect.

4. The answer sheet shall have to be typed or printed.

5. The information, that have to be mentioned in detail, shall have to be enclosed with the application after writing in separate sheet.

6. All pages of the application must be signed and sealed and all signature must be in original.
